Health & Medical Form

Element's Medical Form is a condition of participation in all of our tours and programs. Please download the Medical Form and print it on 8.5" x 11" paper (letter size). Please complete every section of this form and mail or email a signed copy to Elements before your tour or program begins.

Element's Waiver is a condition of participation in all of our tours and programs. Please download the Waiver and print it on 8.5" x 14" paper (legal size). Read the waiver in it's entirety and bring the signed copy to Elements before your tour or program begins.
